Final Sleep Survey

 At the end of the 28 day sleep challenge, it’s so valuable to see how far you’ve come - and importantly, celebrate your successes!
Making change is hard, so the sheer fact you’re trying sets you above and beyond the norm.

To see how far you’ve come, answer the following with reference to the past week,
then compare it against the results from your Baseline Sleep Survey, as completed in week 1.

1. On a scale of 1 to 10, with 10 being the best, rate your:

  • sleep quality overall

  • ability to fall asleep with ease

  • ability to return to sleep, should you wake through the night

  • feelings of refreshment upon waking

  • energy through the day

  • stress and anxiety overall

  • stress and anxiety related to sleep

  • mental clarity

  • memory

  • concentration

  • motivation at work

  • motivation outside of work

  • ability to be present with others

2. Specify the following:

  • how long it takes you to fall asleep

  • how many times you wake through the night

  • sleep length

  • sleep depth

  • one word to describe your sleep

3. Grab your habit tracker and, with last week’s results in mind, write your:

  • Single thing you are the most proud of

  • biggest strength regarding your habits and how this impacted your sleep

  • biggest challenge regarding your habits and how this impacted your sleep

  • How to overcome this challenge in the future

4. Wrap up

  • What was your single biggest ‘win’ from the 28 day sleep challenge, with regards to your sleep?

  • What are the best three changes you’ve seen, specifically regarding your sleep?

  • How has this impacted:

    • Your physical health? And as a result, what are you able to do with greater ease?

    • Your mental health? As a result, how do you feel day to day?

    • Your intimate relationship, family and/or closest friends? How does this change how you impact those around you and ‘show up’ ?

    • Your work? Has your work quality changed and, if so, how? How has this changed your relationships with your clients and colleagues?
